Causes​


Bruises mostly appear from an injury; mainly blunt trauma like bumping by mistake against something (like a door, bed) or from a blow, fall or something hitting you. It can also arise in people who do intense exercise, or from sport injuries., car accidents, hypodermic needle injection and blood drawing.

Next it can occur if a person has low platelet count (thrombocytopenia) or some bleeding disorder.

First among these bleeding disorders is Von Willebrand disease. It is a bleeding disorder in which a person has a low level of a protein called Von Willebrand factor which helps in blood clotting. Due to deficiency of this factor the blood doesn’t clot well. In most cases a person having it is born with this due to an inherited abnormal gene which controls Von Willebrand factor.

Next bleeding disorder is hemophilia. It is a rare inherited genetic disorder in which a person has a deficiency of blood clotting factors and a result the blood doesn’t clot normally. Persons with this tend to bleed for a longer time after an injury as compared to non affected persons. The sufferer also is prone to easy bruising and is at risk of bleeding inside joints (like knees, elbows, ankles) or brain that can be life threatening.

Another bleeding disorder is Christmas disease. It is a genetic disorder in which the body of the sufferer either has decreased levels of factor IX that helps in blood clotting or complete absence of it. It causes impairment in blood clotting. Prolonged bleeding from cuts, or after surgery , unexplained bruising, blood in the urine or stool, bleeding in joints are some of its signs.

Other than these deficiency of clotting factors II, V, VII and X (that helps in blood clotting) can lead to bruising.

Other reasons are leukaemia (blood cancer in which WBC’s I.e. white blood cells in the bone marrow goes on multiplying out of control), ankle sprain, muscle sprain, deep vein thrombosis. Some medicines that make the blood thin like aspirin also can lead to bruises. Deficiency of nutrients including vitamin C, B12 and folate are also linked with frequent bruisings. Some people are naturally prone to get bruises easily. Females are more at risk for bruises as compared to males. Bruises also tend to be more common in elderly people because with age skin gets thin and the tissue that supports the blood vessels also become thin and prone to break easily and capillaries under skin get vulnerable to rupture.

Symptoms​


Initially, skin discolouration appears in case of a bruise. The colour depends upon if it is fresh or if it is under the healing process. Firstly, it is reddish and then it turns bluish, purplish or blackish in some hours. Following it the bruise becomes yellow, green or yellowish brown after a few days. Initially it is red because the blood seeped out is fresh and rich in oxygen. Later its colour changes because pooled blood starts to lose its oxygen and eventually from the by-product compounds that appear from breaking of haemoglobin.

The bruise can be painful and tender to touch. Small bruises are not dangerous and tend to heal easily in a few days. But sometimes bruises can be serious. In some cases bruises can be present in case of fracture needing urgent attention. Sometimes bruises may be indicative of severe internal bleeding that also need urgent medical help. Blackish bruises on the leg may sometimes point towards DVT that can be life threatening. In DVT a blood clot forms in a deep vein usually in the vein of lower legs causing swelling, pain, redness and heat. This clot carries a chance to get detached and travel through blood circulation to the lungs which can be life threatening if not immediately attended.

Excessive swelling around the bruise, numbness of arms / legs, where fracture of bone is suspected, when it occurs without any reason, when they are recurring without any reason, when they don’t heal in a few weeks (around 2 weeks), when they keep growing, when they appear on abdomen, back or face, if a lump (hematoma) form over the bruise and when attended with vision impairment or bleeding from nose or gums or with blood in urine / stool are some of the signs that can be indicative of serious conditions needing immediate medical help.
